How to Access HSE Home Support Services

Accessing HSE Home Support Services is straightforward and designed to ensure that those in need receive timely support

Eligibility Assessment

A Public Health Nurse, GP, or another healthcare professional can provide an information booklet and application form for HSE Home Support Services. You can also download these forms from the HSE website.

Application Submission

Complete the application form and return it to your healthcare professional or local HSE Home Support Office. If you are unable to apply yourself, a relative, GP, or Public Health Nurse can complete it on your behalf.

Care Needs Assessment

After receiving the completed application, the HSE will arrange a Care Needs Assessment to determine the appropriate level and type of support required.
